1. Acceptance of Terms
By accessing our physical facilities, utilizing our digital patient portal, or engaging with our medical staff, you agree to be bound by these Terms of Service. If you do not agree to these terms, you must not use our services.
We reserve the right to modify these terms at any time. Significant changes will be communicated via email or through an alert in the Patient Portal.
2. Medical Disclaimer (Not for Emergencies)
Our digital platforms, telehealth services, and AI-assisted triage tools are NOT designed to handle medical emergencies.
If you are experiencing a life-threatening medical emergency (e.g., severe chest pain, difficulty breathing, severe bleeding, or loss of consciousness), immediately call your local emergency services (such as 911 in the US) or proceed to the nearest hospital emergency room. Do not rely on our digital communication channels for immediate emergency response.
3. Appointments & Cancellations
We strive to respect the time of both our patients and our medical providers.
- Arrival: Please arrive at least 15 minutes prior to your scheduled appointment time to complete any necessary intake documentation.
- Late Arrivals: If you arrive more than 15 minutes late, your appointment may be shortened or rescheduled to avoid disrupting subsequent patients' care.
- Cancellations: We require at least 24 hours' notice for appointment cancellations or rescheduling. Failure to provide adequate notice may result in a late cancellation fee.
4. Financial Responsibility
As a patient, you are ultimately responsible for all charges incurred during your care. While we assist in billing insurance providers and verifying coverage:
- Co-pays & Deductibles: All co-payments, deductibles, and non-covered services must be paid at the time of service.
- Insurance Disputes: If your insurance provider denies a claim or determines a service is not medically necessary, the outstanding balance becomes your direct responsibility.
- Payment Plans: For extensive treatments, structured payment plans may be arranged through our billing department prior to the commencement of care.
